Late Arrivals

Cancellations and Rescheduling

Patients are seen by appointment. Therefore, if you arrive late, the appointment must end as scheduled in fairness to other patients scheduled. Full fee still applies.

If there is a pattern of canceled or missed appointments this will need to be discussed in session. If there is a set appointment time that is reserved for you it may be forfeited at this point.

If you need to reach me between appointments, please call (917) 521-6730 and leave a message and I will get back to you within the same day. Always state your name and telephone number. If it is anything extensive that you need to discuss it would be best to discuss during your session time.

I do not provide urgent, crisis or emergency services. In the event of an urgent need outside of an appointment, please contact your local emergency room, crisis intervention services, or call 911.

Confidentiality

Aside from certain exceptions required by law, information shared between patient and provider is strictly confidential. In the event that there is a person/entity with which you need me to communicate we will need to discuss this ahead of time. If agreed, then an Authorization for Release of Information will need to be completed for each person/entity with whom you would like me to communicate as I can only release information with your written permission.

However, please note, that as a mental health professional I am a mandated reporter under my license and am required to release information without your permission in the following instances:

· A patient is in imminent danger of harming him or herself or another person;

· A patient communicates a serious threat of physical violence against another person;

· Suspected or informed abuse or neglect of a minor, elder or dependent individual;

· A parent or guardian is unable to adequately provide for a child’s basic needs;

· Records are ordered to be released by a judge or court;

· and or as otherwise required by law.
